PRE-CRUISE PAPERWORK
There are 2 parts to this paperwork.
1. In order for us to prepare for your charter, we would appreciate it if you would complete our online Cruise Application form. In order to keep the information secure, our office staff will send you a private link to your Cruise Application. Please contact Mike or Joeli at explore@naia.com.fj
2. The second part is the Liability Release and Diver or Non-Diver Liability Agreement. This part must be signed and sent by post or fax. If it is within 60 days of your charter, please use fax.

CREDIT CARD AUTHORIZATION FORM
NAI'A & PACIFIC ISLANDS TRAVEL offer a built-in 3% discount for cash or check payment. This 3% discount is already built into the package rates that are advertised on all of NAI'A & PACIFIC ISLANDS TRAVEL price sheets and also on the NAI'A & PACIFIC ISLANDS TRAVEL websiteS. However, clients who choose payment via credit card are freely waiving this built-in 3% discount. By this waiver they will pay our normal retail price for their booking -- which is 3% higher than the special cash-discount rate advertised on NAI'A & PACIFIC ISLANDS TRAVEL price sheets & on the internet. Please see our Booking information for further details.
If you are making a payment using a credit card, please print and fill out the appropriate Credit Card Authorization Form and fax it to us in Fiji: (+ Country Code 679)-345-0566.
